Is It Time For Hospice?
For those that have never experienced hospice personally, they may not have a full appreciation for the extensive range of services we offer and our support for patients AND caregivers. However, for many, even the word “hospice” can be a challenging topic. We have developed a special program to help provide an introduction to the collection of services – even if a patient is not quite ready.
Palliative Pathways
Our unique Palliative Pathways Programs offers a team to support you and your loved ones. It is designed to support individuals who have a serious illness but may not be ready for hospice (emotionally or medically) or who have been discharged from hospice. We connect patients and/or caregivers with education, support, and community resources as appropriate for your situation.
Anyone can refer a person to Palliative Pathways by calling (833) 936-8154. The Palliative Pathways Associate will connect with the prospective client and his/her family.
Palliative Care Services
Palliative Care Services is a program run independently from Hearth Hospice that is committed to caring for those with a serious illness with understanding and compassion. Not be to be confused with hospice care, their focus is to serve patients who are struggling with advanced illness or complex medical conditions with a goal of helping to improve quality of life.
